Before You Arrive

Our office requires a referral from another physician before an appointment can be made. Once you have an appointment a new patient packet will be mailed to you, which you will complete and bring with you to your appointment.

New Patients should arrive 30 minutes prior to your appointment and established patients should arrive 15-20 minutes prior to your appointment.

HIPAA

We are HIPAA compliant and you will be given a copy of our privacy policy. If you have any questions about our HIPAA privacy policies, contact our office 541-868-9233 and ask for the HIPAA compliance officer.

Billing

Any insurance co-pays must be paid at the time of your visit. Payment forms accepted are check, cash or credit card (Visa or MasterCard). Remaining balances after insurance has paid are due within 30 days. If you are uninsured or are not able to pay your balance in full, contact our business office to work out a mutually acceptable payment plan. The business office telephone number is (541) 485-7137. Insurance

Be sure to bring your insurance card with you. Eugene Springfield Nephrology Associates provides care for patients in most managed care and standard insurance plans. You should check with your insurance company if you have questions about whether we are on their panel of providers. All of our physicians accept Medicare assignment, but you will be required to pay the balance after Medicare or your supplement has paid.
